The Medical Assistance Personal Care Program helps qualified chronically ill and disabled persons to remain in their homes by providing assistance with activities of daily living.
The Medical Assistance Personal Care Program provides management and funding for Personal Care Providers who give help with activities of daily living in clients’ homes. Personal care providers also provide household services directly related to medical need. The Personal Care Program can help with:

Holders of an active federal Medical Assistance card

Elderly, chronically ill, or disabled

Under a physician’s care

In need of help with daily activities
Personal Care Providers Approved by the Health Department
All personal care providers complete an initial application, submit references, and agree to a criminal background check. In addition, they are certified to be in good physical health and are given a TB test by the health department. The health department works in close contact with clients and providers to ensure that providers meet the standards set forth in the plan of care.
A Registered Nurse Monitors On-Going Care
A registered nurse from the health department evaluates clients’ unique needs for care and works with each client to develop a personalized plan of care to meet those needs. The nurse locates a personal care provider and oversees the quality of care and service they provide for clients. As part of the ongoing plan, the nurse visits with clients and the personal care provider throughout the year and updates each client’s medical history annually.
