St. Mary's County Health Department provides assistance to aged and functionally disabled adults who are at risk for being placed in an institution. Our goal is to help people remain in the community or in the least restrictive living situation where they can function at the highest possible level of independence and personal well-being. Clients receive a comprehensive evaluation to determine their needs and to identify services available to help the individual.
Services
Referrals
Referrals are accepted from any concerned individual by phone, fax, mail or in person. Referrals for pre-admission screening and resident reviews generally come from hospitals, nursing facilities, private physicians or other health care providers.
Fee
There is no charge for evaluations.
