Birth Certificate Applications Now Accepted Through Mail

LEONARDTOWN, MD (June 19, 2020) – The St. Mary’s County Health Department (SMCHD) is now accepting birth certificate applications through the mail.  While the health department is maintaining public health services for the community, the Leonardtown building is closed to walk-in clients from the public at this time. Birth certificates, however, may be requested via mail.

Certified copies of birth certificates for residents born in the State of Maryland may be obtained by:

  • Downloading the birth certificate application at: smchd.org/birth-certificates
  • Completing the application and including a copy of a photo identification (parent/guardian ID required for children)
  • Submitting payment in the amount of $25 (check or money order made out to the St. Mary’s County Health Department)
  • Mailing all of the above required documents and payment to:
