Health Department Seeking Primary Care Providers for Million Hearts Collaborative

LEONARDTOWN, MD (October 30, 2014) – The St. Mary’s County Health Department is seeking local primary care practices to implement clinical quality improvement, including team-based care strategies for adult blood pressure management. Six primary care practice sites in St. Mary’s County who participate in the St. Mary’s County Million Hearts Collaborative and implement clinical quality improvement measures promoting cardiovascular health for their adult patients will be awarded $6,000 each. Funding has been made available for this initiative through the Maryland Department of Health and Mental Hygiene (DHMH). Selected provider practices will utilize team-based care strategies for blood pressure control and community-based resources to assist in optimizing patients’ health. The project additionally promotes identification and management of pre-diabetes and diabetes.

To be considered, interested primary care practices need to respond to the announcement posted on the St. Mary’s County Health Department website at www.smchd.org/rfp by the November 21, 2014 deadline.
